If you are scheduled for one of our advanced imaging studies you may wish to complete some additional history information that we will require prior to your exam. Please feel free to complete and print the following information at your convenience and bring it with you to your appointment. If you are unsure your appointment requires this information please contact our scheduling department at 623-847-2000 for assistance.
